Portfolio Evaluation Checklist

Ultimately, the goal of home education portfolios is to simply show signs of progress.  

In getting ready for your evaluation, please provide the following to help me ensure this requirement is being fulfilled:

Requirements (by Florida law):

_____ Log of educational activities. A list of ongoing curriculums, activities, field trips, programs, etc. You may include a brief statement of the educational purpose of each activity.

_____ Interview with the child (as the child is able) answering basic questions about his/her school year. Emailed or video responses are allowed (faces can be omitted from the video for privacy). Note: Questions will be emailed directly to the parent and are age/academic level specific.

_____ Three or more samples of work from each subject. Please show samples from beginning, middle, and end of the evaluation year. These samples may include (but are not limited to) pictures of activities, worksheets, assessments, projects, book reports, writing samples, etc.   

Other optional but beneficial documents showing “signs of progress”:

 _____ A brief summary of the child’s development and progress throughout the evaluation year.

_____ Book log. Include all all books read throughout the year. Please include the author. Optional beneficial information: number of pages, subject, literature genre, integrated activities (how the book connected with other subjects and ELA skills).

_____ Online course completion documentation (if applicable)

_____ Treatment plans and/or evaluations from a therapist (ABA, OT, SLP, Vision, etc.) (if applicable)

_____ Pictures or videos of the child engaging in skill developing activities. Pictures of the child’s face may be covered for privacy.
